กรณีหนังสือเดินทางหมดอายุ/สูญหายในนิวซีแลนด์ และจำเป็นต้องเดินทางกลับประเทศไทยด่วน ผู้เดินทางสามารถขอเอกสารเดินทางฉุกเฉิน (Emergency Travel Document) เพื่อเดินทางกลับประเทศไทยเป็นการเร่งด่วน โดยจะต้องแสดงตนและนำหลักฐานยืนยันตัวตนมายื่นต่อสถานเอกอัครราชทูตฯ เพื่อขอรับเอกสารฯ

1. ขั้นตอนการดำเนินการ
     1.1 ดาวน์โหลดและกรอกแบบฟอร์ม คำร้องเอกสารเดินทางฉุกเฉิน/คำร้องหนังสือเดินทาง
     1.2 จองคิวนัดหมายวัน/เวลาที่จะเข้ามายื่นคำขอเอกสารเดินทางฉุกเฉิน (คลิ๊กเพื่อจองคิว)
           - หากเป็นกรณีเร่งด่วน หรือไม่สามารถจองคิวนัดหมายในระบบได้ กรุณาติดต่อแจ้งขอทำเอกสารเดินทางฉุกเฉินมาที่ [email protected]  
     1.3 ในวันที่นัดหมาย กรุณานำใบคำร้องที่กรอกเรียบร้อยพร้อมเอกสารประกอบ (รายละเอียดตามข้อ 2) มายื่นด้วยตนเองที่สถานเอกอัครราชทูตฯ ตามเวลาที่นัดหมาย

2. เอกสารประกอบ
        2.1 หนังสือเดินทางเล่มปัจจุบัน (หากหนังสือเดินทางสูญหาย ให้นำใบแจ้งความทรัพย์สินสูญหายที่ตำรวจออกให้ - Lost/Stolen Property Report - และสำเนาหนังสือเดินทางเล่มที่หายมาแสดงแทน)
        2.2 บัตรประจำตัวประชาชน (หากบัตรประจำตัวประชาชนสูญหาย ให้นำหลักฐานอื่น ๆ ที่มีรูปถ่ายแสดงตัวบุคคลและหมายเลขประจำตัวประชาชน ซึ่งออกให้โดยหน่วยงานของรัฐบาลมาแสดงแทน)
        2.3 สำเนาทะเบียนบ้าน (หากมี)
        2.4 บัตรโดยสารเครื่องบินที่ยืนยันกำหนดการเดินทาง
        2.5 หากเป็นผู้เยาว์ (อายุต่ำกว่า 20 ปี) จะต้องมีบิดา-มารดามาแสดงตนที่สถานเอกอัครราชทูตฯ เพื่อให้การยินยอมให้บุตรเดินทาง หรือหากบิดา-มารดาอยู่ที่ประเทศไทย ผู้เยาว์จะต้องมีหนังสือที่บิดา-มารดาลงนามที่สำนักงานเขต/อำเภอเพื่อให้ความยินยอมให้บุตรเดินทาง (หนังสือให้ความยินยอมให้บุตรเดินทาง) และเอกสารประจำตัวของผู้ปกครอง (สำเนาหนังสือเดินทาง/บัตรประจำตัวประชาชน/สำเนาทะเบียนบ้าน) มาแสดงประกอบการขอเอกสารเดินทางฉุกเฉินด้วย
        2.6 หากเป็นผู้เยาว์ (อายุต่ำกว่า 20 ปี) ที่ยังไม่เคยมีหนังสือเดินทางไทยมาก่อน จะต้องนำสูติบัตร / หนังสือรับรองสถานที่เกิด มาแสดงเทน
          
3. ค่าธรรมเนียม สถานเอกอัครราชทูตฯ ไม่คิดค่าธรรมเนียม ในการออกเอกสารเดินทางฉุกเฉินและได้รับเอกสารในวันเดียวกับที่ทำเอกสารเดินทางฉุกเฉิน (Emergency Travel Document : ETD)  

หมายเหตุ: เอกสารเดินทางฉุกเฉินมีอายุใช้ได้เพียง 1 เดือน และไม่สามารถใช้เดินทางไปยังประเทศที่สาม เนื่องจากเป็นเพียงเอกสารที่ออกแทนหนังสือเดินทางใช้ในการเดินทางกลับประเทศ ไทยได้ครั้งเดียว
 

An emergency travel document may be obtained in case a passport is lost or stolen and there is an urgent need to travel back to Thailand. The applicant must submit the application in person at the Royal Thai Embassy.

1.  Application Procedures 
     1.1 Complete the following form
             Request for a Emergency Travel Document (Download)   
      1.2 Book an appointment to submit your application (Click to book your appointment) 
            - Applications must be submitted in person only
            - If you need to travel urgently or if you were not able to book an appointment online, please send an email to [email protected]
      1.3 On the date of the appointment, please bring your application forms together with all required documents (see below) to the Embassy.

2.  Required documents
     3.1 Current Thai Passport (In case of lost/stolen passport, a police report of your lost/stolen passport, to be filed at a local police station, must be presented)
     3.2 Thai Identity Card (In case of lost Thai identity card, other forms of valid Thai photo identification issued by Thai authorities must be presented)
     3.3 Thai House Registration (if applicable)
     3.4 Copy of return air ticket or confirmed travel itinerary
     3.5 Applicants under 20 years of age must be accompanied by their parents.  If their parents are in Thailand, a letter of consent to travel signed by both parents (or legal guardian) at a district office in Thailand will be required.  Copies of Thai passports / ID cards and house registration of the parents or guardian must be submitted together with the letter of consent.
     3.6 Applicants under 20 years of age who have never previously held a Thai passport must present a Thai Birth Certificate to the Embassy.
      
3.  Fee There is no application fee for an application for a Certificate of Identity   

Note: An emergency travel document is valid for only 1 month and is not valid for traveling to a third country as it is issued solely for the purpose of a one-time trip to Thailand
